Personal Care

The goal of the Personal Care Program is to provide personal care service for seniors 60+ who are confined to their home and unable to attend to their personal care.

 

• An outreach worker will come to the home to complete an assessment on the senior
• A care plan is developed for each client that will help him/her to remain independent in his or her home.
• The Personal Care attendant goes to the client's home once a week for a two-hour period to bathe and perform other personal hygiene according to the care plan.
• A yearly reassessment of the needs of the client is conducted and the care plan is revised accordingly.
• The Personal Care attendant receives 40 hours of training each year and the program is certified through the state.
• Suggested donation is $5.00 per person per month.
