You should bring this information with you to enrollment events or provide it with your completed application that you mail to us.

____ Proof of income for each member of household (current tax form, Social
Security Benefit letter or current bank statement)

____ List of medications, directions/strengths, prescribing doctor, and cost of drug.

____ Summary of prescription drug costs

____ Name, phone, and address for primary physician

____ Insurance card for proof of insurance (Medicare, Medicaid, private, etc.)

____ Information on your assets, living expenses, and medical expenses
