ALZHEIMER'S SERVICES RESOURCE GUIDE

SERVICES AND PROGRAMS AVAILABLE
TO THE ELDERLY

Elderly Protective Services(EPS): Deals with claims of abuse and self-neglect to citizens 60 and older. They respond to reports, plan a remedy, stabilize the situation and refer cases to proper legal authorities, if necessary. (225) 342-9722, Toil Free (in-state only) 800-259-4990. Shreveport Region (318) 676-5200 or 800-259-5284. Shreveport Region serves: Bienville, Bossier, Caddo, Claiborne, Desoto, Natchitoches. Red River, Sabine, and Webster parishes.
Outreach. Caddo Council on Aging. Provides Individual Needs Assessment to explain services available and to enroll in their various programs. (318) 632-2090 or www.caddocouncilonaging.org. Call for requirements. Donations are requested for most programs. Outside of Caddo parish, check with your parish Council on Aging (numbers listed
below). Many offer similar programs.
Family Caregiver: Provides a caregiver of a senior for respite, sitter and personal care.
96 hr/year.
Homemaker: Provides trained worker to perform tight household tasks for those confined to their home.
Legal Services
Meals on Wheels: Noon-time meal will be delivered five days a week to limited number of home-bound persons.

Nursing Home Ombudsman: A trained volunteer makes an investigation and resolution of complaints made by or on behalf of residents in a long-term care facility.

Personal Care: Trained worker will provide personal care weekly for the elderly confined at home and unable to attend to their own personal care.
Telephone Reassurance: Regular telephoning to a client to provide comfort and/or help. Emergency response systems are available.
Transportation: Services are provided from 8:00 a.m. to 4:30 p.m., Monday - Friday for individuals who have no other means to travel to doctor's office, hospital, grocery store, and other life supporting services.
