Because the high cost of
prescription drugs can be a great burden
for many Louisiana senior
citizens, our state is offering resources to help seniors obtain much-needed
aid. The
mission of the Louisiana SenioRx Program is to link qualifying senior citizens with pharmaceutical assistance
programs. SenioRx does not supply free or discounted drugs, but provides
assistance for accessing and applying for aid.
SenioRx
Can Provide Assistance to:
ü
Seniors age 60 and older who are legal Louisiana residents.
ü
Seniors with an income
below 300% of the poverty level:
o
$27,930
annually ($2,327 monthly) for a
one-person household
o
$37,470
annually ($3,122 monthly) for a
two-person household.
ü
Seniors with a chronic illness taking prescribed daily medications.
ü
Seniors who do not have insurance covering medications.
ü
Seniors who have not
voluntarily cancelled state/federal prescription drug
programs or a private reimbursement plan within 6
months.
SenioRx does not provide
assistance for obtaining short-term medicines to meet acute needs.

enroll in SenioRx,

You should bring this information
with you to enrollment events or provide it with your completed application
that you mail.
____ Proof of income for each member of household
(current tax form,
Social Security Benefit letter or
current bank statement)
____ List of medications, directions/strengths,
prescribing doctor,
and cost of drug
____ Summary of prescription drug costs
____ Name, phone, and address for primary physician
____ Insurance card for proof of insurance (Medicare,
Medicaid, private, etc.)
____ Information on your assets, living expenses, and
medical expenses
